We've got you covered. You can find the download for the Utah Conditional Final Release form template here. You may have skipped over it as the title doesn't include the term "conditional." But the state of Utah regulates their lien waiver forms, and provides only two under Utah Code §38-1a-802; a progress payment waiver, and a final payment waiver. Both of which are conditional. Utah mechanics lien statutes do not provide for unconditional lien waivers.
